Painful trigeminal neuropathy is damage to one or more branches of the trigeminal nerve leading to sensory disturbances (hypoesthesia, paresthesia, dysesthesia) or neuropathic pain within its distribution.
It should not be confused with trigeminal neuralgia, which is a paroxysmal pain condition without clear sensory deficit.
The exact prevalence of painful trigeminal neuropathy is not well established, as it is a significantly rarer condition than trigeminal neuralgia. It is estimated that trigeminal neuropathies account for fewer than 10% of neurologically-driven facial pain cases.
In the general population, the incidence of facial sensory neuropathies, including trigeminal nerve damage, is approximately 1–5 per 100,000 persons per year, although data vary depending on etiology and study population.
Main Causes
The epidemiology of trigeminal neuropathy is closely tied to its etiology, as it is not a distinct disease entity but rather a symptom complex resulting from various mechanisms of nerve damage.
The most common cause is mechanical trauma, particularly in the course of:
- dental procedures such as tooth extractions, implantation, or inferior alveolar nerve blocks,
- facial trauma or surgery involving the jaws and oral cavity,
- neoplastic damage, including cerebellopontine angle tumors, meningiomas, schwannomas, or metastases to the skull base,
- nerve injury following aesthetic medicine procedures.
These neuropathies typically have a gradual onset and present with unilateral sensory symptoms.
Infections can also lead to trigeminal neuropathy, most commonly:
- herpes zoster (VZV),
- Lyme disease,
- syphilis,
- tuberculosis,
- HIV.
In such cases, neuropathic pain is often burning in character and accompanied by paresthesias.
In autoimmune and inflammatory diseases such as multiple sclerosis, sarcoidosis, or systemic lupus, neuropathy may be a focal symptom or an early manifestation of the underlying disease.
Diagnosis
Diagnosis is based on a detailed history, neurological examination, and analysis of risk factors (e.g., dental procedures or infections).
Assessment of superficial sensation, reflex evaluation, and pain testing allow the extent of deficit to be determined.
To confirm nerve damage, imaging studies are performed, primarily MRI (3T with 3D sequences), which enables assessment of the nerve's course and any compressive or inflammatory changes.
In some cases, neurophysiological studies (e.g., somatosensory evoked potentials) are also useful.
Differential diagnosis includes classical trigeminal neuralgia, myofascial pain, temporomandibular joint disorders, and other cranial neuropathies.
Treatment
Treatment depends on the cause. In post-traumatic and iatrogenic cases, early intervention is most important: pharmacotherapy supporting nerve regeneration (e.g., vitamin B12, centrally-acting analgesics such as gabapentin or pregabalin), and sensory rehabilitation.
In infections, causal treatment is applied (e.g., antiviral therapy, antibiotics), and in tumor cases, neurosurgical or oncological management.
Increasingly, however, emphasis is placed on a holistic approach. Chronic neuropathic pain requires not only pharmacological treatment but also psychological support, cognitive behavioral therapy (CBT), relaxation techniques, and nervous system-targeted physiotherapy. Patient education, learning pain-coping strategies, and attention to sleep, diet, and physical activity can significantly improve quality of life for people with neuropathy.
