That is why it is so important to understand how emotions form in response to different signals from the body — and how we can influence them.
What are automatic thoughts?
Automatic thoughts are brief, instantaneous mental comments that:
- Appear in a fraction of a second after a situation occurs
- Often stem from past difficult experiences with pain
- Can be highly negative or catastrophic
- Have an enormous impact on emotional reactions and muscle tension
- Can increase the sensitivity of the nervous system
Examples:
- A gentle breeze on the face → "Oh no, this could trigger an attack..."
- Mild discomfort → "It's starting, it's going to get worse..."
- Stronger discomfort → "I won't be able to talk, this might provoke the pain."
These thoughts appear automatically — and they are not your fault.
But you can learn to notice them and reframe them, so they don't add suffering on top of the pain.
Thoughts create emotions — and emotions influence pain
Our brain responds not only to what is happening, but primarily to how we interpret it. Especially if in the past the pain was intense, sudden, or hard to control.
Example:
Imagine you're drinking warm tea and feel a slight tingling near your cheek.
If you think: "It's probably nothing, it'll pass in a moment" — you are likely to remain emotionally calm.
If you think: "This is definitely the start of an attack, I'm in for a nightmare" — fear appears, tension builds, you start scanning your face, and stress takes over.
This matters enormously, because emotions and muscle tension intensify neuropathic pain.
A stressed nervous system = a more sensitive nervous system.
3 steps to understanding your reaction
This is a simple method that helps you notice what exactly happens between a stimulus and your experience of pain and emotion.
Step 1: What happened?
A stimulus, a situation, a signal from the body — large or small.
Step 2: What went through my mind?
The automatic interpretation, usually unconscious. Often it is a fear of recurrence, an expectation of the worst, or an assessment of the situation through the lens of previous pain episodes.
Step 3: How did it affect me?
Emotions, physical reactions, muscle tension, behavior.
Example:
1. What happened?
During a conversation, you felt a brief "jolt" in the jaw area.
2. What went through your mind?
"I need to stop talking, because it's probably going to get worse. This could be the start of an attack."
3. How did it affect you?
Emotions: fear, tension, a sense of threat.
Body: facial and neck muscles tighten automatically, breathing speeds up, you become hypervigilant to every signal from your face.
Behavior: you stop the conversation, avoid speaking further, monitor the pain.
These reactions are completely natural — but they can unnecessarily amplify nerve hypersensitivity.
Why does working with thoughts help specifically in neuralgia and neuropathy?
Because with this type of pain:
- The nervous system becomes more vigilant and reactive
- The brain is quicker to search for potential "threats"
- Negative thoughts can intensify reflexive tension in the muscles of the face and neck
- And this in turn can provoke or increase pain
Noticing and softening automatic thoughts:
- Reduces muscle tension
- Decreases vigilance and face-monitoring
- Lowers nervous system arousal
- Helps restore a sense of control
- Can reduce the intensity of flare-ups
This is not a method for instantly eliminating pain, but it is a method for limiting what pain tends to "build around itself" — namely fear, tension, and stress.

Sometimes the emotion appears first (for example, fear), before you even have time to think.
But it is your thoughts that determine whether that emotion calms down or spirals.
And a spiraling emotion = greater tension = greater nervous system sensitivity = greater pain.
