Severe facial pain is one of the most distressing and confusing symptoms in medicine. Many patients immediately hear "trigeminal nerve," but in practice, not every pain in this region is classical trigeminal neuralgia.
Very often the source of suffering turns out to be temporomandibular joint (TMJ) disorder, and sometimes also trigeminal neuropathy.
Moreover, these conditions can mimic each other or even coexist, making diagnosis difficult and delaying effective treatment.
Why It Feels Like "Nerve Pain"
Shared innervation
The temporomandibular joint and the muscles of mastication are innervated primarily by the mandibular branch of the trigeminal nerve (V3). This means that any inflammation, overload, or damage to the TMJ produces pain in the "territory" of the trigeminal nerve. The patient feels pain in the cheek, temple, or ear — exactly where trigeminal neuralgia pain is also felt.
Referred pain and sensitization
Chronic TMJ dysfunction can lead to peripheral and central sensitization. This is a phenomenon in which sensory fibers of the trigeminal nerve become hyperreactive — responding more intensely and more frequently than normal. The result is chronic pain with features of neuropathic pain.
Not classical neuralgia, but possible neuropathy
Classical trigeminal neuralgia (TN) typically arises from vascular compression of the nerve root in the brainstem — TMJ dysfunction alone will not cause this.
However, chronic inflammation or microtrauma in the TMJ can trigger secondary trigeminal neuropathy — that is, continuous, burning pain, sometimes with sensory disturbances.
How to Distinguish What Is Actually Causing the Pain
Trigeminal Neuralgia
- Pain: sudden, piercing, "like an electric shock."
- Duration: seconds to 2 minutes.
- Triggers: touch, brushing teeth, speaking, a gust of air.
- Between attacks: usually pain-free.
Trigeminal Neuropathy
- Pain: constant, burning, stinging.
- Additional symptoms: numbness, tingling, reduced sensation.
- Often follows dental procedures, aesthetic medicine treatments, jaw surgery, trauma, or infections.
TMJ Disorder
- Pain: dull, chronic, worsening with jaw movement.
- Additional symptoms: clicking in the joint, popping, limited mouth opening, tenderness of the muscles of mastication.
- Radiation: to the cheek, ear, temple.
Where to Seek Help
- Neurologist — if pain is paroxysmal, electric, unbearable; suspected classical neuralgia.
- Dentist specializing in TMJ disorders.
- Oral and maxillofacial surgeon — if clicking, limited mouth opening, or pain on chewing is present.
- Dental physiotherapist — if overload of the muscles of mastication or bruxism is suspected.
- In unclear cases, the best results come from collaboration between multiple specialists — neurologist, dentist, and physiotherapist.
Diagnostic Workup
- Detailed history and clinical examination — questions about pain character, triggers, and duration.
- Brain MRI with trigeminal nerve assessment — when classical neuralgia is suspected.
- MRI or CBCT of the TMJ — if TMJ disorder is the issue; TMJ ultrasound is also recommended.
- Neurological examination for sensory disturbances (typical of neuropathy).
Treatment of TMJ-Related Pain
Conservative management:
- Physiotherapy,
- Exercises for the muscles of mastication,
- Occlusal splints,
- Stress management techniques.
Additionally:
- Anti-inflammatory medications (NSAIDs).
- In selected cases: dental treatment, intra-articular injections.
