Postherpetic trigeminal neuralgia represents one of the most complex forms of neuropathic pain affecting the face. This condition develops as a consequence of reactivation of the varicella-zoster virus (VZV), which after primary infection remains dormant in nervous system structures — primarily in the trigeminal ganglia and dorsal root ganglia. The virus remains present in the body for life and can reactivate after many years. This process is most commonly associated with aging or immune compromise, though atypical courses are increasingly observed in younger individuals as well.

The classical presentation

The classic picture of herpes zoster is a unilateral, painful vesicular rash spreading along a dermatome — the area of skin innervated by a specific nerve. When the trigeminal nerve is involved, the clinical picture can be considerably more complex. Particularly problematic are situations where pain dominates with minimal skin or mucosal changes, or when changes are atypical. In such circumstances, diagnosis may be delayed and patients subjected to numerous diagnostic and therapeutic procedures for weeks or months.

A classical example is a 58-year-old man who presented with severe left-sided forehead pain preceding the appearance of vesicular rash above the left eye. Symptoms were unilateral, limited to the territory of the first trigeminal branch (ophthalmic, V1). He described significant hypersensitivity of the affected area, including allodynia — even a cool breeze from a fan clearly intensified the pain. Physical examination revealed typical vesicles above the left eye without auricular involvement, fever, or other significant findings. Laboratory results were normal. The clinical picture corresponded to acute herpes zoster of the first trigeminal branch.

When the presentation is atypical

Not all cases of trigeminal herpes zoster have a typical skin presentation. This is especially true for changes limited to oral mucous membranes, sparse skin lesions, or situations where pain clearly precedes vesicle appearance. In such cases, the condition may be confused with dental pain, sinusitis, atypical facial neuralgia, or primary headache syndromes.

Particular attention is needed for cases in younger individuals who fall outside the classic epidemiological profile. One such example involves a young patient whose initial symptoms appeared as headaches, weakness, and right-sided cheek and gum pain. Initially, sinusitis was suspected and antibiotics were started, but the patient's condition rapidly worsened.

Over subsequent weeks, a long and complicated diagnostic journey ensued. Multiple dental consultations and procedures were performed — caries treatment, root canal therapy, imaging — yet symptoms persisted. The patient repeatedly reported erythema and vesicular changes on the gum, accompanied by extremely intense pain described as a sharp instrument being driven into the tooth or gum. The pain had clear neuropathic characteristics: constant, stabbing, intensifying over time.

A particularly significant moment was when vesicular oral changes appeared and herpes zoster was suspected — but initially dismissed due to atypical localization. Only the family physician ultimately recognized VZV reactivation as probable and initiated antiviral treatment.

Why diagnosis matters

Comparing the classical case with the atypical presentation clearly demonstrates how different the clinical spectrum of trigeminal herpes zoster can be. While obvious skin changes in V1 facilitate diagnosis, changes limited to the gum combined with dominant pain symptoms led to multistage, initially misdirected diagnostics.

In the atypical case, the patient subsequently developed severe headaches, dizziness, nausea, syncope, and neurological symptoms. Extensive testing including MRI, CT, cerebrospinal fluid analysis, and immunological workup revealed no definitive abnormalities. Meanwhile, chronic pain gradually intensified and became refractory to successive treatment attempts.

How postherpetic pain develops

During viral reactivation, inflammation develops and damages neural structures responsible for receiving and transmitting pain signals. As a result, the nervous system can become hypersensitive and send excessively strong pain signals. Over time, changes may also involve the central nervous system, causing the brain to perceive even minor or normal stimuli more intensely. This may explain why pain persists for months or years even after visible disease symptoms resolve.
