Sometimes I wake up thinking, *"Today will be a good day,"* yet the pain still appears. Other times, before I've fully opened my eyes or my brain has completely woken up, the pain is already there at full intensity, draining my energy from the very beginning of the day. There are also days when nothing seems to suggest that a flare-up is coming, yet the pain suddenly intensifies and fills my head, turning the entire day into a struggle for every breath. These fluctuations in symptoms are very common, and there are several scientific explanations for why they happen.
What does the science say?
- Sleep and sleep quality have a major impact on pain sensitivity. Poor sleep increases pain intensity and lowers your ability to tolerate pain.
- Stress and emotional states influence the nervous system. Anxiety, emotional tension, and chronic stress are often associated with more severe pain.
- Central sensitization, a process in which the nervous system becomes overly responsive to stimuli, helps explain why even a minor trigger, such as wind or a light touch, can cause intense pain.
- Weather and changes in atmospheric pressure have been linked to pain flare-ups in some people. For example, cold wind or drops in barometric pressure may trigger attacks or worsen continuous pain in certain patients.
- Changes in the brain and emotional networks that occur with chronic pain strengthen the connection between emotions and pain perception.
How does it work?
Imagine your nervous system as a radio with an extremely sensitive speaker. When everything is working normally, you hear only what you need to hear. But when you're sleep deprived or under significant stress, the volume gets turned up, and even the quietest background noise becomes overwhelming.
- Poor sleep interferes with your body's ability to recover and increases sensitivity to stimuli. As a result, even a gentle touch or a change in temperature may feel much more painful. Review studies have consistently linked sleep disturbances with greater pain intensity.
- A stressful day brings muscle tension, especially in the jaw and neck, elevated cortisol levels, and increased alertness. Together, these changes lower your pain threshold.
- Weather can also play a role. For some people, cold wind, damp air, or falling barometric pressure increase symptoms. Not everyone experiences this, and sensitivity varies from person to person.
What does this mean if you're living with trigeminal neuralgia or painful trigeminal neuropathy?
- Pain fluctuations are normal. They are part of the condition itself and do not necessarily mean that your treatment has stopped working.
- Different triggers can add up. Poor sleep, stress, and cold wind together may turn a manageable day into a very difficult one.
- It is not your fault. Your nervous system is reacting, and some people simply have a more sensitive "volume control" than others.
Gentle, practical steps that may be worth considering

These strategies do not promise an immediate cure, but they may help you gain a little more control over the ups and downs of chronic pain.
Sleep *(I'll be writing much more about this)*
- Try to maintain a consistent sleep schedule by going to bed and waking up at the same time each day.
- If you're struggling with sleep, talk to your doctor. Improving sleep quality often reduces pain intensity.
Stress and tension
- Take short, regular breaks throughout the day. Even two to five minutes of breathing or relaxation exercises can help. Personally, they've helped me a great deal. Sometimes just one or two slow, calm breaths are enough to reduce muscle tension.
- Practice simple relaxation techniques or mindfulness for two to ten minutes a day, without putting pressure on yourself. Research suggests these approaches help regulate the nervous system.
Your body
- Gently relax the muscles of your face, jaw, and neck with slow, gentle stretching exercises. I'll be sharing some ideas for these exercises in future posts.
- During flare-ups, try to avoid sudden or intense triggers whenever possible, such as exposing your face to cold wind without protection.
Weather
- If you've noticed that weather affects your symptoms, keep a scarf, hat, or face covering nearby on windy days. It may also help to have a backup plan for days with low barometric pressure, such as scheduling less demanding activities, if your work and daily responsibilities allow.
Support
- If the cycle of pain, stress, and poor sleep keeps repeating, consider seeking support from a healthcare professional, such as a neurologist, psychologist, or physical therapist. Asking for help is not a sign of weakness. It's simply another way of breaking the cycle.
Remember that difficult days do not diminish the value of the better ones. Fluctuations in pain intensity are a natural part of these conditions and have biological explanations, from sleep quality and stress levels to weather changes and the mechanisms of central sensitization. Knowledge alone will not make pain disappear in an instant, but it can provide a roadmap. Once you begin to recognize the factors that "turn up the volume" on your pain, it becomes easier to plan small steps that may help soften its waves.
If it's difficult to identify when your pain worsens or what triggers it, don't put pressure on yourself. Living with pain is already a heavy burden. Being patient and compassionate with yourself is far more important than trying to observe everything perfectly, no matter where you are on this journey.
