Treating trigeminal neuralgia is challenging, and in many cases may be only partially effective. Treatment success depends on many factors: misdiagnosis, inappropriate treatment, doses that are too low, and delays in surgical intervention.
Treatment Strategy
Isolated pain attacks do not always require pharmacological treatment.
Exacerbations require a multidirectional approach: pharmacotherapy, psychological support, and adequate hydration.
In cases of severe attacks, hospitalization may be necessary for medication adjustment and dose optimization.
1. Long-Term Pharmacotherapy — First-Line Treatment
Carbamazepine
The drug of choice in classical trigeminal neuralgia.
- Dose: 200–1200 mg/day, divided, gradually increased.
- Mechanism: sodium channel blockade, activation of GABA receptors.
- Efficacy: 70–75% of patients.
- Possible side effects: drowsiness, dizziness, nausea, hyponatremia, allergic reactions (Stevens-Johnson syndrome).
- Monitoring of blood count and sodium levels is required.
Oxcarbazepine
- Similar efficacy to carbamazepine with a better tolerability profile.
- Dose: 600–1800 mg/day (exceptionally up to 2700 mg).
- Side effects: rash, hyponatremia, bone demineralization.
2. Second-Line Medications
- Lamotrigine — used in refractory cases, dose: 25–400 mg/day, gradually increased.
- Baclofen — muscle relaxant that also relieves pain, dose: 60–80 mg/day, 3–4 times daily.
- Pimozide — an antipsychotic, rarely used due to side effects, dose: 2–12 mg/day.
3. Third-Line and Adjunctive Medications
- Gabapentin — inhibits excitatory neurotransmitter release, dose: 1800–3600 mg/day.
- Pregabalin — similar to gabapentin, dose: 150–600 mg/day, well tolerated.
- Topiramate — blocks sodium channels, dose: 100–500 mg/day.
Additionally, topical treatments are used: lidocaine patches, capsaicin cream, intranasal medications (sumatriptan, tizanidine), and novel substances under investigation, such as vixotrigine.
Important note regarding contraception: Some anticonvulsants (e.g., carbamazepine, oxcarbazepine, phenytoin) may reduce the effectiveness of hormonal contraception. If taking these medications, discuss additional or alternative contraceptive methods with your doctor.
4. Surgical Treatment
Indicated for patients refractory to pharmacotherapy — after trialing at least 3 medications.
Surgical methods:
- Trigeminal nerve lesioning (transection, alcohol, radiofrequency, cryotherapy).
- Gasserian ganglion lesioning.
- Trigeminal nerve root lesioning.
- Microvascular decompression (MVD) — the most commonly performed procedure, effective in 80–90% of patients, removing vascular compression from the nerve.
- Advantages: improved quality of life, relatively low invasiveness.
- Disadvantages: cost, possible transient sensory loss or paresthesias.
5. Multimodal Approach
Treatment combines pharmacology, psychological therapy, rehabilitation, nerve blocks, cryotherapy, acupuncture, and transcutaneous electrical nerve stimulation.
In postherpetic neuralgia, early use of antiviral medications and effective pain control are important to reduce the risk of chronic pain.

Trigeminal neuralgia requires an individualized, stepwise approach:
- Diagnosis — pain assessment, MRI, exclusion of dental causes.
- Pharmacological treatment — medication adjustment, monitoring of effects and side effects.
