Trigeminal nerve pain is one of the most distressing and difficult-to-bear types of pain. It can take different forms:
- Neuralgia — sudden, piercing, "electric shock-like" pain lasting seconds to minutes, often triggered by speaking, chewing, touching the face, or brushing teeth.
- Neuropathy — continuous, burning, stinging pain, frequently accompanied by numbness, hypersensitivity, or altered sensation.
These two presentations require similar specialists, but not always the same treatment. Here is a step-by-step guide.
First Steps — Where to Seek Help
1. Neurologist — the primary specialist.
- Will conduct a history and neurological examination.
- May order an MRI of the brain with an appropriate protocol to rule out vascular compression of the nerve, tumors, or other lesions.
- Will propose pharmacological treatment (e.g., carbamazepine, oxcarbazepine — the classic first-line medications).
2. Dentist / oral and maxillofacial surgeon — if the pain seems "dental."
- Important, because neuralgia is often confused with toothache, and many patients see a dentist first.
- Allows dental causes to be ruled out.
3. Anesthesiologist / pain medicine specialist (pain clinic)
- Responsible for advanced pain management: nerve blocks, injections, radiofrequency procedures.
- Pain clinics offer interdisciplinary care, particularly when pain is refractory to medications.
4. Neurosurgeon / stereotactic neurosurgery centers — if conservative treatment fails.
- Possible procedures: microvascular decompression (MVD), percutaneous procedures (radiofrequency, balloon compression, glycerol injections), and stereotactic radiosurgery (e.g., Gamma Knife).
- In analyses, MVD provides the best long-term outcomes, but the choice of method depends on patient age, cause of pain, and overall health.
Important Distinction: Neuralgia vs. Neuropathy Treatment
The above procedures are typically used in trigeminal neuralgia — where we have paroxysmal, piercing, brief pains usually related to vascular compression of the nerve root.
Why?
- MVD works when the cause of pain is a vessel compressing the nerve. In neuropathy (e.g., post-traumatic, post-dental surgery, post-herpetic), this mechanism is absent, so the effect would be poor.
- Percutaneous procedures and Gamma Knife also target interruption of pain conduction in classical neuralgia. In neuropathy, where pain is chronic, burning, and related to nerve damage itself, effectiveness is significantly lower.
In neuropathy, the main treatment approaches are:
- pharmacotherapy (gabapentin, pregabalin, baclofen),
- nerve blocks, local injections, sometimes botulinum toxin,
- supportive therapies (chronic pain management, psychological support, rehabilitation),
- in exceptional, very difficult cases — neuromodulation (e.g., stimulation of the nerve or pain centers).
Pharmacological Treatment Summary
- First-line: carbamazepine, oxcarbazepine.
- Other options: gabapentin, pregabalin, baclofen — selection depends on tolerance and clinical presentation.
- Botulinum toxin (Botox) — growing body of evidence for effectiveness in some patients, particularly refractory cases; however, still considered an additional or experimental option in some centers.

Pain psychology demonstrates that the brain actively responds to pain: it processes nerve signals, and additionally, emotions, stress, and tension can amplify pain perception. The greater the psychological burden, the stronger the pain experience may become.
Physiotherapy, relaxation techniques, yoga, mindfulness — they do not eliminate the cause but help reduce tension and cope with chronic pain.
