Clinical picture
The most characteristic symptom is unilateral, stabbing or piercing pain localized in the throat, neck, jaw angle, or ear. Many patients also report a foreign body sensation in the throat. Symptoms usually intensify during swallowing, yawning, speaking, or head movements.
Pain may radiate to the ear, base of the tongue, face, or submandibular area. In rarer cases, compression of the carotid arteries occurs, leading to vascular symptoms dependent on head position.
A clinical case — the importance of pain characteristics
The clinical course of Eagle syndrome is well illustrated by a patient presenting with unilateral neck pain related to swallowing and head movements.
Initially, the patient suspected an ear infection, but more detailed history revealed several features inconsistent with a typical inflammatory process. There was no fever or other signs of infection, while the pain had a clearly mechanical character — appearing during specific activities and provoked by movement.
The patient precisely localized the source in the anterior neck on one side. Particularly significant was pain intensification during swallowing and head rotation. This symptom pattern is characteristic of Eagle syndrome because it suggests irritation of anatomical structures associated with the styloid process or stylohyoid ligament.
Why diagnosis is difficult
Eagle syndrome is among conditions often overlooked in differential diagnosis. This results primarily from nonspecific symptoms and their similarity to far more common diseases.
Key differential diagnoses include glossopharyngeal neuralgia, temporomandibular joint disorders, chronic ENT conditions, and dental pathology. An additional difficulty is that symptoms often appear only during specific movements or activities, meaning their significance may be overlooked by both patient and physician.
Establishing the relationship between pain and swallowing, head movement, or pressure on specific neck areas is therefore crucial.
Diagnosis
Diagnosis is based on combining history, physical examination, and imaging. During examination, the physician may attempt to reproduce pain through palpation of the tonsillar area or lateral pharyngeal wall. In selected cases, a diagnostic block with local anesthetic is helpful.
The diagnostic standard remains CT, particularly with 3D reconstruction. This allows assessment of styloid process length, extent of stylohyoid ligament calcification, and anatomical relationships with surrounding structures.
If vascular compression is suspected, CT angiography performed in different head positions may be a valuable supplement.
Treatment
Treatment can be conservative or surgical. Conservative management includes analgesics, NSAIDs, corticosteroids, and (for neuropathic components) neuromodulatory medications.
In some patients, local anesthetic injections prove effective, serving both diagnostic and therapeutic functions.
When severe symptoms persist, surgical treatment involving shortening or removal of the styloid process may be considered. This procedure eliminates the cause of mechanical irritation and represents the most definitive therapeutic method.
