Painful post-traumatic trigeminal neuropathy (PTNP) is a chronic neuropathic pain affecting the face and oral cavity that develops as a result of damage to the trigeminal nerve. This nerve is responsible for sensation in the face (mouth, gums, teeth, lips, jaw) and for chewing movements.
The pain can appear after dental procedures (most commonly after wisdom tooth extraction, implants, root canal treatment, or local anesthesia) or after facial trauma.
The incidence of PTNP ranges from 1.5% to 13% following procedures or injuries.
Most commonly it:
- Involves the mandibular nerve (lower jaw)
- Occurs in women, particularly between the ages of 40 and 60
Symptoms
Symptoms are highly variable and may include:
- Burning, stabbing, searing, lancinating pain
- Tingling, numbness, a sensation of a "foreign body"
- Hypersensitivity to touch (pain triggered by light contact)
- Spontaneous pain or pain triggered by speaking, eating, or brushing teeth
- Temperature disturbances (sensations of cold or heat)
- Swelling without an apparent cause
In some patients, the pain is constant; in others, it occurs in episodes.
The condition often leads to:
- Sleep problems
- Anxiety and depression
- Significantly reduced quality of life
Why is PTNP difficult to diagnose?
There are no universally accepted, definitive diagnostic criteria. Diagnosis is based on:
- History of nerve injury
- Clinical symptoms
- Sensory testing
- Imaging studies
The diagnosis of PTNP is consistent with the criteria of the International Headache Society (ICHD-3) and the International Classification of Orofacial Pain (ICOP).
A misdiagnosis can lead to:
- Unnecessary procedures
- Worsening of pain
- Years of patient suffering
What tests are performed?
Depending on the case:
- Sensory testing (touch, temperature, pain)
- Quantitative sensory testing (QST)
- Panoramic X-rays, CBCT (assessment of bone and nerve pathway)
- Magnetic resonance imaging (MRI)

How pain develops
After nerve damage, the following occur:
- Inflammation
- Excessive nerve excitability
- Abnormal pain impulses
- Changes in the brain responsible for chronic pain
Over time, pain can become self-sustaining even without further nerve damage.
Treatment
Treatment depends on the duration of symptoms and the degree of nerve damage.
1. Early treatment (best outcomes)
- Rapid removal of the cause (e.g., an implant compressing the nerve)
- Sometimes a short course of corticosteroids to reduce inflammation
2. Pharmacological treatment (most common)
- Antiepileptic drugs (e.g., gabapentin, pregabalin)
- Antidepressants (TCAs, SNRIs)
Effectiveness is limited — approximately 10–15% of patients experience meaningful relief.
3. Topical treatment
- Lidocaine gels
- Capsaicin (with caution)
- Specialized protective oral splints
4. Botulinum toxin (Botox)
- Can significantly reduce pain
- Effects last several months
- Used when other methods have failed
5. Surgical treatment
- Only in selected cases
6. Psychological support
- Very important in chronic illness
- Helps cope with pain, anxiety, and depression
What is the prognosis?
Unfortunately, in 50–70% of patients, pain is long-lasting or permanent, and the longer pain persists, the more difficult it becomes to treat.
However, in many individuals over time:
- Pain diminishes
- It becomes better controlled
- It stops dominating daily functioning
The longer pain persists, the more it engages the nervous system, which is why treatment can become more challenging. At the same time, the nervous system retains the capacity for adaptation, and appropriately selected treatment, sensory rehabilitation, psychological support, and coping strategies can meaningfully improve quality of life.
Many patients learn to:
- Identify factors that worsen pain
- Reduce its impact on sleep, work, and relationships
- Regain a sense of control over their body and daily life
What is critically important: early recognition and prompt, appropriate treatment increase the chance of improvement and even partial symptom resolution. That is why it is so essential that pain is not dismissed and that patients are not left alone with it.
Although living with pain can be difficult, life is not over. Real methods exist to reduce suffering, regain agency, and gradually build a good quality of life — even if the path is gradual and requires support.
