For twelve years, pain has been my teacher. Not the kind you choose — but the kind that shows up uninvited.
At first, I despised it...
I despised it for taking nearly my entire life... right at the beginning of adulthood. For stealing lightness, plans, a sense of safety. For bringing fear where there was once curiosity.
It was hard. The kind of hard that only those understand who wake up in the morning and their first thought isn't "What will I do today?" but "Will I be able to do anything today?"
Who learn to function with tension in the jaw, the throat, the temples, the neck...
Who hear "you look fine" when their body is screaming otherwise.
I felt anger, a sense of injustice, helplessness. I saw nothing in pain but an enemy.
Only after years did I understand that although I didn't choose this lesson, I could choose the way I'd move through it.
Thanks to this teacher, I've changed. I became stronger, calmer, more patient. I started listening to my body. More carefully choosing what I feed it.
It responded with strength and agility. It responded with cooperation.
But one thing I'm still learning — not to endlessly return to the past. To the moment when my pain-free life ceased to exist. When I fell down the rabbit hole of medical abstractions. When the system that was supposed to help didn't understand. Didn't believe. And when it did — it was already too late.
I was left alone with the pain. On the other side of the mirror.
And when I return to those moments, I feel anger, frustration, helplessness.
That's when I begin to breathe. Because breath is not just air. It's an anchor, a bridge, a signal sent deep into the nervous system:
"Things are different now. You are safe now."
A conscious, calm exhale:
- Lowers cortisol levels
- Reduces adrenaline release
- Increases vagal nerve activity
- Regulates heart rhythm
- Relaxes the diaphragm
- Tells the brain that the alarm can be turned off
Breath restores context. It separates the past from the present and stops the neurochemical avalanche before it takes the wheel. And this is exactly where the real "here and now" begins.
What does "being present" mean with chronic pain?
Chronic pain changes how the entire body functions.
It is not merely a signal from a damaged nerve.
It becomes an experience encompassing:
- The autonomic nervous system
- Breathing
- Muscle tension
- Emotions
- The way the brain interprets past and future
In a world of instant solutions — techniques, protocols, biohacks — a question arises:
Does the first step really have to be change?
Or perhaps what's needed first is simply seeing?
1. Breath as a mirror of the nervous system
The way we breathe reflects the state of the autonomic nervous system.
This system has two main branches:
- Sympathetic — mobilization, fight, flight
- Parasympathetic — regeneration, digestion, calm
With chronic pain — especially in the face (one of the most densely innervated areas) — the sympathetic system often remains overactive. The body operates in vigilance mode.
What appears:
- Shallow, shortened inhale
- Holding air mid-breath
- No full exhale
- Tense diaphragm
- Clenched jaw
- Stiff throat
- Tense abdomen
Breathing stops being the rhythm of life. It becomes a control mechanism. Before any technique, it's worth asking:
*What is my breath telling me about my sense of safety?*
Notice your breath and consider this question.
2. Why we return to the past
Logically, we know the past is over.
Neurobiologically — it's more complex.
When a painful memory appears:
- The amygdala activates
- Prefrontal cortex activity decreases
- The stress axis engages
The result: cortisol and adrenaline begin circulating. Breathing quickens, jaw and diaphragm tension appears, and anger takes hold of emotions...
The brain reacts as if the threat were happening now.
This is not weakness — it's a survival mechanism.
3. Anger as biology
Anger during returns to the past is mobilization.
Neurobiologically:
- It increases noradrenaline
- It raises muscle tension
- It temporarily restores a sense of agency
But underneath it often lie:
- Helplessness
- Fear
- Sadness
- A sense of injustice
With chronic pain, the nervous system is already sensitized. Every return to the past reinforces vigilance.
4. What conscious breathing actually does
Breathing works because it is simultaneously automatic and voluntary.
We can use it to influence the autonomic nervous system.
An extended exhale:
- Activates the vagus nerve
- Increases heart rate variability
- Lowers amygdala arousal
- Increases prefrontal cortex activity
- Restores neurochemical balance
But it does something even more important.
It creates a micro-pause between stimulus and response. It restores choice. It returns the steering wheel to the present moment.
This isn't an escape from the past — it's regulation of biology that prevents the past from ruling the present moment.
Our reactions, tension, and anger make biological sense. The nervous system was trying to protect us the best way it knew how.
And breath — that gentle, natural, quiet movement of the diaphragm —
is like a door left ajar between the world of alarm and the world of presence.
Every time you step through it, you teach your body that things are different now.
