Persistent Idiopathic Dentoalveolar Pain (PIDAP), historically known as atypical odontalgia, is a chronic tooth or dentoalveolar pain condition without an identifiable dental cause.

Unlike typical toothache, the pain often persists despite appropriate dental treatment and is thought to involve neuropathic pain mechanisms or altered pain processing within the nervous system.

Diagnosis is one of exclusion and requires careful dental and neurological assessment. Management is multidisciplinary and may include neuropathic pain medications, psychological support, patient education, and avoiding unnecessary dental procedures.
