Treatment of trigeminal neuralgia should be individualized and adjusted to symptom severity and the underlying cause.
First-line therapy usually includes carbamazepine or oxcarbazepine. Other options may include lamotrigine, baclofen, gabapentin, pregabalin, topiramate, and selected adjunctive therapies. Surgical procedures such as microvascular decompression may be considered when medication is ineffective or poorly tolerated.
The most effective care often combines medication, interventional procedures when appropriate, rehabilitation, psychological support, and multidisciplinary pain management.
